Having trouble submitting the form?

If the form says required fields are missing even though you’ve filled them in please try the following:

• Tap into each field and retype a character (this helps the form recognise your input)
• Avoid using autofill and enter your details manually
• Refresh the page and try again
• If possible, try a different browser or device
